Active Directory : User last logon

Hello,
Chances are report is already REAL somwhere - however I haven`t managed to find it in #finishedreprots.
So basically i`m looking for simple report what would return following values from active directory:

-username
-First name
-Last name
-Department
-Status (active, disabled, locked etc)
-Last time user logged into domain
-Last time user changed PSW (optional)

Status and last login time are not scanned from AD. For the logon time you can use the logon time scanned by Lansweeper. However, by default Lansweeper will only scan which user was logged in at the time of the Lansweeper scan. If you use LsPush with a logon script, you can get accurate login data.
